In the past years, many participants of SHARE allowed us to tak e physical health measurements lik e their grip and lung strength. This year, to more precisely assessdisease risk , we would lik e to collect a few blood drops using just a finger prick as it is done daily by millions of diabetic people. We would be very grateful if you wouldagree to participate. This is an important part of the study, as the analysis of blood samples will tell us a lot about the health of the population. I will explain more to youabout it. Yet, this measurement is absolutely voluntary. If you do not wish to have your finger prick ed, we will leave it out and continue the interview without the biomedicalpart.Before we begin, I would lik e to have you read and sign this consent form.

IN THE PAST YEARS, MANY PARTICIPANTS OF SHARE ALLOWED US TO TAK E PHYSICAL HEALTH MEASUREMENTS LIK E THEIR GRIP AND LUNG STRENGTH. THIS YEAR, TO MORE PRECISELY ASSESSDISEASE RISK , WE WOULD LIK E TO COLLECT A FEW BLOOD DROPS USING JUST A FINGER PRICK AS IT IS DONE DAILY BY MILLIONS OF DIABETIC PEOPLE. WE WOULD BE VERY GRATEFUL IF YOU WOULDAGREE TO PARTICIPATE. THIS IS AN IMPORTANT PART OF THE STUDY, AS THE ANALYSIS OF BLOOD SAMPLES WILL TELL US A LOT ABOUT THE HEALTH OF THE POPULATION. I WILL EXPLAIN MORE TO YOUABOUT IT. YET, THIS MEASUREMENT IS ABSOLUTELY VOLUNTARY. IF YOU DO NOT WISH TO HAVE YOUR FINGER PRICK ED, WE WILL LEAVE IT OUT AND CONTINUE THE INTERVIEW WITHOUT THE BIOMEDICALPART.BEFORE WE BEGIN, I WOULD LIK E TO HAVE YOU READ AND SIGN THIS CONSENT FORM.

As I mentioned, I'd lik e to collect a small sample of your blood. I will clean your finger with an alcohol wipe, then using a small instrument I will prick your finger and collectenough blood to fill at least three small circles on the collection card. I have received a special training to conduct this measurement. If you want, you can also prick yourfinger by yourself.Do you understand the directions for this measurement?

AS I MENTIONED, I'D LIK E TO COLLECT A SMALL SAMPLE OF YOUR BLOOD.
